Job summary

LiveWell Homecare Agency is seeking a licensed Physical Therapist for a facility-based role reporting to the Director of Rehab. You will assess patients, develop recovery plans, deliver individualized therapy, and collaborate with the care team.

You will educate patients and caregivers to promote independence, focus on mobility and wellness, and adapt to patient needs while upholding compassionate, evidence-based care.

Qualifications

• Valid Physical Therapy license or eligibility in the practice state(s).
• Master's degree in Physical Therapy or Bachelor’s degree plus 5 years progressive PT experience. Foreign degrees must be evaluated for U.S. equivalency.
