Job summary

Keizer Nursing & Rehabilitation Center in Keizer, Oregon, seeks a licensed Physical Therapist to evaluate patients, create individualized plans, and guide therapeutic exercises. Full-time, part-time, and PRN/on-call shifts are available with flexible scheduling.

You’ll collaborate with an interdisciplinary team, educate residents and families on safe equipment use, and ensure timely documentation and billing.

Qualifications

• Licensed in Oregon or graduate of an approved Physical Therapist program or actively working toward licensure.
• Graduate of a CAPTE Accredited Physical Therapy Education Program.
• Experience in post-acute care or similar healthcare environment preferred.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans
• Provide therapeutic exercises, functional training, and pain management
• Educate residents and families on safe use of equipment and exercise techniques
• Collaborate with interdisciplinary care teams to promote progress and independence
• Accurately complete documentation and billing in a timely manner
