Job summary

Synchrony is seeking a Physical Therapist (PT) to provide a full range of physical therapy services at Springhurst Health Campus in Indiana. Responsibilities include assessment, treatment planning, and interdisciplinary care in a supportive environment.

The role requires an active PT license in Indiana and CPR certification (preferred). Experience ranges from 0 to 1 year, with orientation and ongoing professional development offered.

Qualifications

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehab needs.
• Develop and implement personalized treatment plans with exercises and modalities.
• Educate patients and families on exercise techniques and body mechanics.
• Maintain accurate documentation of progress, plans, and outcomes in compliance with policies.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Perform other duties as assigned.
