Job summary

Glendale Transitional Care Center is seeking a licensed Physical Therapist (PT) to evaluate and treat residents, develop care plans, and coordinate with the care team. The PT will assess strength, mobility and respiration, administer tests, and implement treatments under federal guidelines.

Requirements include a state license in California and 1 year of clinical experience preferred; new graduates are welcome.

Qualifications

• Bachelor/Master/Doctorate in Physical Therapy required or preferred.
• One year of clinical experience preferred; new graduates welcomed.
• Current California Physical Therapy license.

Responsibilities

• Evaluates, plans and administers treatment to residents within PT scope of practice and federal guidelines.
• Administers and interprets tests and measurements for muscle strength, ROM and related functions.
• Provides direct resident treatment according to established care plans.
• Maintains confidentiality of patient records.
• Conducts home assessments to support safe discharge from care.
• Completes required forms and documents per policy and regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.

• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care..
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
