Position Summary:

Performs initial and ongoing assessments of patient's condition. Establishes a care plan tailored to identified problems, involving the patient according to their capabilities and desires. Plans, prepares, and executes individualized physical treatment programs to maintain, improve, or restore physical function, alleviate pain, and prevent dysfunction. Performs therapy interventions using standard physical therapy techniques suitable for the patient's age and condition. Supervises physical therapy assistants, students, and support staff by assessing competence, delegating tasks, and maintaining communication. Administers treatments involving physical agents, equipment, moist packs, lamps, and ultrasound machines. Evaluates, fits, and adjusts prosthetic and orthotic devices, recommending modifications. Documents goals, progress, and plans for reevaluation.

Minimum Qualifications:

• Education: Bachelor's Degree Required; Doctorate of Physical Therapy - Required
• Licenses: Physical Therapist License in Kentucky
• Certifications: Basic Life Support (BLS) within 30 days of hire
