Job summary

Monroe Health & Rehab Center is seeking a Physical Therapist to join our skilled nursing facility therapy team. You will provide progressive, individualized care to seniors, aiming to restore independence and meet therapy goals in a supportive, team-driven environment.

This full-time role offers a $3,000 sign-on bonus, competitive hourly wages of $40.00–$50.00, and a comprehensive benefits package including PTO and CEUs.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Must maintain an active, unencumbered PT license for the state of employment.
• Must possess basic computer skills and familiarity with electronic health records.

Responsibilities

• Restore patients to optimal physical function in a SNF setting.
• Instruct patients in maintaining function post-discharge through education and independent exercise.
• Collaborate with physicians and therapy team to meet goals and provide high-quality care.

Position Summary:

The primary function of your job is to return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.

Education/Qualifications:

• Employee must possess a bachelor’s degree or greater in Physical Therapy from an accredited college or university.

• Must maintain an active, unencumbered license granted from state of employment.

• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Shift & Wages:
