Job summary

Casa is seeking a Physical Therapist to help residents regain range of motion, manage pain, and return to daily living. You’ll assess needs, build individualized treatment plans, and guide mobility exercises as part of a collaborative care team.

Active Indiana PT license and graduation from an accredited program required; long-term care experience preferred. You’ll work with residents, families, and staff to support recovery and improve quality of life.

Qualifications

• Active Indiana professional license as a Physical Therapist.
• Graduate of an accredited Physical Therapy program.
• Compassionate, patient, and able to think critically under pressure.
• Strong communication skills with residents, families, and staff.
• Long‑term care experience preferred.

Responsibilities

• Review medical history and assess each resident’s condition and needs.
• Evaluate a resident’s ability to perform daily tasks and movements.
• Build individualized treatment plans with clear, achievable goals.
• Guide residents through exercises that improve mobility and relieve pain.
• Recommend and train residents on mobility equipment such as walkers and wheelchairs.
• Educate families on how to support recovery at home.
• Track progress against goals and prepare reports for providers and insurers.
• Work with physicians, nurses, therapists, and social workers as part of the care team.
