Job summary

Interim HealthCare of West Palm Beach is seeking a PRN/Home Health Physical Therapist to deliver one-on-one care in patients' homes in Florida. You’ll assess, treat, and progress plans aimed at restoring mobility, independence, and quality of life.

This role emphasizes autonomy, flexible scheduling, and meaningful patient connections within a trusted home health team. You must be a licensed PT in FL with at least 1 year in home health, CPR certified, and capable of navigating state and federal

Qualifications

  • Graduate of an accredited Physical Therapist Program and active PT license in FL.
  • Minimum of 1 year of physical therapy experience in home healthcare.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion.

Responsibilities

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality.
  • Work as part of a home health team with RN, LPN, CNA, HHA, OT, and SLP to implement the plan of care.
  • Observe movements, conduct assessments, document progress and modify the care plan as needed.
  • Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
  • Educate patients and families on the plan of care, goals, exercises, and self-care.

As a Home Health Physical Therapist, here’s what to expect:

  • Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
  • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient’s plan of care and goals
  • Observe movements, conduct assessments, document progress and modify care plan as needed
  • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients on their plan of care, established goals, exercises, and self-care
  • Coach family on the plan of care, therapy goals and safely assisting with exercises

A few requirements for Home Health Physical Therapists:

  • Graduate of an accredited Physical Therapist Program and active PT license in FL
  • Minimum of 1 year of physical therapy experience in home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion
