Job summary

Interim HealthCare Inc. is seeking a licensed Physical Therapist to provide home health services across Indiana. The role focuses on evaluating patients, designing personalized treatment plans, and helping them regain mobility in a home setting.

The position offers flexible FT/PT scheduling, competitive pay with benefits, and a commitment to patient-centered care with 1:1 attention. CPR/First Aid and reliable transportation are required for site visits.

Qualifications

• Current unrestricted PT license to practice as a Physical Therapist in the state of Indiana.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.
• At least 1 year of professional PT experience in home health or similar setting.

Responsibilities

• Evaluate and treat patients in home health settings to improve mobility and reduce disability.
• Develop and administer treatment plans using exercises, stretching, manual therapy, and equipment.
• Educate patients and families on rehabilitation and activities for health and independence.
• Collaborate with other providers to optimize patient outcomes.

Physical Therapist, PT - Home Health - FT/PT

Responsibilities

• Evaluate and treat patients suffering from physical disability or lack of mobility due to injury, disease, or surgery in a home care environment
• Establish and administer a treatment plan using exercises, stretching, manual therapy, and equipment to manage patients' pain and increase mobility
• Educates patients and family members in rehabilitative care and activities necessary to promote health, safety, and independent living
• Collaborate with other providers servicing your client(s) to reach the best possible patient outcomes

Qualifications

• Licensure: Current unrestricted license to practice as a Physical Therapist (PT) in the state associated with this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations
• One (1) year of professional experience practicing as a Physical Therapist (PT) in a home health or similar setting.
