Job summary

Harmony Health and Rehab in Georgia seeks a licensed Physical Therapist to evaluate residents, develop individualized treatment plans, and deliver therapies aimed at improving mobility and reducing pain. You will assess function, set goals, and track progress in a team-based skilled nursing setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license.
• CPR certification required (or to be obtained on hire).
• Prior experience in skilled nursing or long-term care preferred.

Responsibilities

• Conduct comprehensive PT evaluations and develop personalized care plans.
• Deliver PT treatments including exercises, gait training, balance activities, and mobility.
• Monitor and document progress and adjust plans as needed.
• Collaborate with nursing, OT, speech therapy, and physicians for holistic care.
• Educate residents and families on therapy programs and safety measures.
• Comply with regulatory, licensing, and infection control standards.

The Physical Therapist (PT) will be responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions to help residents improve or restore mobility, reduce pain, and achieve their highest level of physical function.

Physical Therapist Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals.

• Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility.

• Monitor and document residents' progress, response to treatment, and adjust plans as necessary.

• Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care.

• Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence.

• Comply with all regulatory, licensing, and facility policies including documentation and infection control standards.

Physical Therapist Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current and valid Physical Therapist license.

• CPR certification required (or must obtain upon hire).

• Prior experience in a skilled nursing or long-term care setting preferred.
