Job summary

Harmony Health and Rehab in Georgia operates a 120-bed skilled nursing environment. The Physical Therapist evaluates residents, develops treatment plans, and delivers interventions to improve mobility, reduce pain, and function independently.

You will collaborate with nursing, occupational therapy, speech therapy and physicians to provide holistic care and educate residents and families on therapy programs. Compliance with regulatory standards is required as part of daily clinical practice.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license.
• CPR certification required (or must obtain upon hire).
• Prior experience in a skilled nursing or long-term care setting preferred.
• Strong communication, clinical, and organizational skills.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals.
• Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility.
• Monitor and document residents’ progress, response to treatment, and adjust plans as necessary.
• Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care.
• Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence.
• Comply with all regulatory, licensing, and facility policies including documentation and infection control standards.
