Job summary

Elm Management seeks a compassionate Physical Therapist to provide quality rehabilitation services to residents in a skilled nursing setting. You will evaluate residents, develop individualized treatment plans, and deliver therapy to improve mobility, strength, balance, and independence.

You will collaborate with nursing staff and physicians, document progress, educate residents and families, and ensure compliance with policies and standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of employment.
• Experience in skilled nursing/long-term care/rehabilitation is preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate residents' physical abilities and functional needs.
• Develop individualized treatment plans based on residents' conditions and goals.
• Provide physical therapy treatments according to established care plans.
• Perform therapeutic exercises and functional mobility training.
• Assist residents with improving strength, balance, coordination, and range of motion.
• Monitor residents' progress and modify treatment plans as appropriate.
• Document evaluations, treatments, progress, and other required information accurately.
• Communicate with nursing staff, physicians, families, and other members of the interdisciplinary team.
• Educate residents and caregivers on exercises, mobility techniques, and safety measures.
• Follow facility policies, regulatory requirements, and applicable professional standards.
