Qualifications

• Minimum 1 year of experience in a skilled nursing and rehabilitation setting (or equivalent).

Responsibilities

• Provide expert physical therapy services to patients in a skilled nursing and rehabilitation setting, including evaluations, treatments, and discharge planning.
• Develop and implement individualized treatment plans to address patient needs and goals.
• Collaborate with interdisciplinary team members to ensure comprehensive patient care.
• Conduct patient assessments, set goals, and track progress towards achieving optimal outcomes.
• Maintain accurate and detailed records of patient treatments and progress.
• Participate in education and training of rehabilitation staff and students.
• Stay current with industry developments and best practices through ongoing education and professional development.
