Role Overview:

The Physical Therapist provides direct rehabilitation care in a skilled nursing facility, focusing on long-term support and short-term rehabilitation for diverse patient populations with orthopedic, neurological, and cardiopulmonary needs.

Responsibilities Include

• Developing and implementing personalized treatment plans aligned with physician referrals and regulatory standards
• Assessing patient progress, adjusting goals, and overseeing care provided by PTAs and support staff
• Documenting patient care accurately and utilizing therapy software effectively
• Collaborating with interdisciplinary teams to promote patient recovery and well-being
• Participating in community health initiatives and maintaining professional relationships
• Ensuring safety, confidentiality, and compliance with policies, laws, and ethical standards

Qualifications & Skills

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT); licensure in applicable state
• CPR certification; experience preferred but new grads are welcome
• Proficiency in Microsoft Office and good communication skills
• Ability to make independent decisions, demonstrate good body mechanics, and work as part of a team
• Strong understanding of post-acute care practices, laws, and regulations
