Post Rehab is seeking a full-time Physical Therapist (PT) to join the Civita Care Center at Cheshire, CT. You will work with residents in both short-term rehabilitation and long-term care, focusing on improving their mobility, strength, balance, and overall function.

This role involves evaluating residents, creating treatment plans, and collaborating with a multidisciplinary team. The position offers comprehensive benefits including medical, dental, and vision insurance, along with opportunities for professional development.

Qualifications

• Must have an active Physical Therapist license.
• Strong communication and clinical reasoning skills are essential.
• Skilled nursing experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and establish individualized treatment plans.
• Provide treatment focused on mobility, gait training, transfers, balance, and pain management.
• Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members.

Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team. This position offers the opportunity to work with residents in both short‑term rehabilitation and long‑term care while helping them improve mobility, strength, balance, and overall function.

Position Responsibilities:

• Perform physical therapy evaluations and establish individualized treatment plans
• Provide treatment focused on mobility, gait training, transfers, balance, strengthening, endurance, and pain management
• Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members
• Monitor resident progress and adjust treatment plans as appropriate
