Job summary

Alternate Solutions Health Network is seeking a compassionate Physical Therapist (PT) to join our Home Health team. The PT will provide skilled therapy in patients' homes, develop care plans, and collaborate with an interdisciplinary team to maximize independence.

Position includes a sign-on bonus of $10,000, travel through East Canton, Canton, Canal Fulton, Massillon, and East Sparta, and a Mon-Fri 8am-5pm schedule. Mercy Medical Homecare supports professional growth.

Qualifications

• Must have at least 1 year of PT experience; acute care setting required.
• Home health experience preferred and valid PT license in state of employment.
• Strong clinical assessment, critical thinking, and patient education skills; ability to work independently.

Responsibilities

• Provide skilled home health PT services as ordered by the attending physician.
• Perform evaluations including OASIS assessments and develop individualized plans of care.
• Modify treatment plans based on patient progress and clinical findings.
• Educate patients and caregivers on disease management, home exercise programs, and use of assistive devices.
• Communicate patient status and recommendations to physicians and care team; supervise PTAs as permitted.
• Complete all required documentation accurately within timeframes.

The Physical Therapist provides skilled therapy services in patients' homes under physician orders, helping individuals improve mobility, reduce pain, restore function, and maximize independence. The PT develops individualized plans of care, educates patients and caregivers, and collaborates with the interdisciplinary team to achieve exceptional patient outcomes while maintaining compliance with Medicare, state, and agency guidelines.Key ResponsibilitiesProvide skilled home health physical therapy services as ordered by the attending physician.Perform comprehensive evaluations, including OASIS assessments, and develop individualized plans of care.As the treating Physical Therapist, assess patient progress and modify treatment plans based on clinical findings.Deliver evidence-based interventions to improve strength, balance, mobility, endurance, and functional independence.Educate patients and caregivers on disease management, fall prevention, home exercise programs, and the safe use of assistive devices.Communicate patient status, progress, and recommendations to physicians and the interdisciplinary care team.Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and agency policies.Ensure continuity of care through admissions, recertifications, resumptions of care, transfers, and discharges.Complete all required clinical documentation accurately and within agency timeframes, including Start of Care, evaluation, recertification, routine visit, and discharge documentation.Maintain compliance with Medicare, federal, state, and company documentation standards.Participate in case conferences, on-call responsibilities, mandatory meetings, and continuing education.Travel to patient homes according to the daily visit schedule.Perform other duties as assigned.QualificationsMinimum of one (1) year of experience as a Physical Therapist in an acute care setting required.Home health experience preferred.Current, unrestricted PT license in the state of employment.Strong clinical assessment, critical thinking, and patient education skills.Excellent communication and collaboration skills with patients, families, physicians, and healthcare professionals.Ability to prioritize workload, meet documentation deadlines, and work independently.Valid driver's license, reliable transportation, and current auto insurance.Education & LicensureGraduate of an accredited Physical Therapy program.Current license to practice as a Physical Therapist (PT) in the state of employment.Physical RequirementsFrequently walk, stand, bend, reach, and lift up to 50 pounds with or without assistance.Travel daily to patient homes within the assigned service area.Use electronic medical records and other computer systems proficiently.Demonstrate the visual, hearing, and communication abilities necessary to safely perform the essential functions of the Physical Therapist role.Why Join Our Team?Meaningful one-on-one patient careFlexible schedulingSupportive interdisciplinary teamCompetitive compensation and benefitsProfessional growth and continuing education opportunitiesIf you are a compassionate Physical Therapist looking to make a meaningful impact through home health care, we encourage you to apply.
