Job summary

Summa Home Health is seeking a qualified Physical Therapist to provide comprehensive in-home therapy services as ordered by physicians. The PT educates patients and families, ensures safety, and maintains compliant documentation and plan of care in accordance with Medicare guidelines.

The PT will perform OASIS assessments, develop individualized care plans, and supervise PTAs and aides as appropriate. A current PT license and a valid driver’s license are required; home health experience is

Qualifications

  • Licensed Physical Therapist with active license in state of employment.
  • Minimum one year of PT experience; home health experience preferred.
  • Excellent communication and observation skills.
  • Valid driver’s license and auto insurance.

Responsibilities

  • Provide skilled physical therapy in patient's home as ordered by physician.
  • Observe, record, and report patient changes to physician and agency staff.
  • Perform OASIS assessments to develop an individualized plan of care and adjust as needs change.
  • Develop and execute plan of care; treat patients to relieve pain and restore function.
  • Coordinate admissions, transfers and discharges to ensure continuity of care.
  • Educate patient and family on disease/injury and plan of care.
  • Supervise therapy services of PTAs as appropriate.
  • Document all aspects of treatment and assessments per guidelines and timelines.
  • Participate in on-call requirements and mandatory trainings.

Work 2 holidays a year (one winter, one summer).Territory: Macedonia, Twinsburg, Aurora, Brady Lake, Garrettsville, Hiram, Hudson, Kent, Ravenna, Tallmadge, Diamond, South CanalSUMMARYThe Physical Therapist I (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the patient. The PT I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.KEY RESPONSIBILITIESProvides skilled physical therapy in a patient’s home as ordered by the attending physicianObserves, records, and reports patient reactions, changes, and progress to attending physician and any appropriate agency staffPerforms OASIS assessments to develop an individualized plan of care and makes adjustments as needs changeDevelops plan of careTreats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.Arranges the provision of services listed above on an outpatient basis which cannot be given in the patient’s homePromotes continuity of care with appropriate admissions, transfers and dischargesCounsels patient and family on the disease/injury and how to manage in all phases of physical therapyIncorporates patient and family in development of plan of careInstructs patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devicesEnsures patient/family know financial responsibility for needed equipment by utilizing DME department and/or MSWHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersOversees and supervises therapy services of patient provided by PTAs and when appropriate, nurse assistantsEvaluates the POC based on patient response to treatment/medical condition and adjusts as needed.Communicates with the PTAs and home health aides on supervisory visit resultsCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationCompletes chart audits at agency site location as requestedMaintains active PT license in state of employmentCommunicates with scheduler any changes as neededParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsParticipates in orientation/training of new employeesMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresDrives to patient’s primary location per scheduled visit.Daily attendance at assigned visit locations.Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits.
