Job summary

Summit Home Care and Hospice is seeking an experienced Physical Therapist to provide in-home PT services across Indianapolis, Greenwood, Beech Grove, and Avon. Therapies are delivered in patients' homes, with some remote charting and office training available.

The role requires Indiana PT licensure, a PT degree, and a preference for 2+ years of home health experience. Strong communication skills and ability to work independently are essential.

Summit is also proud to offer AI-Assisted clinical documentation to streamline charting and free ECUs.Physical Therapist (PT):Hours: Monday - Friday; Standard Business Hours & Flexible SchedulingAnnual Salary: $95,000 - $115,000 per YearSign-On Bonus: $10,000 (Paid in Installments)Additional Earnings: $2,000 Quarterly Bonus for Maintaining an Average Productivity of 90%)Coverage Location: Indianapolis (Central), Greenwood, Beech Grove, and AvonWork Setting: On the Road for Patient Visits & Training at the Local Office or RemotelyResponsibilities:Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.Assess and evaluate therapeutic, rehabilitative/functional status.Develop, implement and direct physical therapy programs that focus on rehabilitation.Evaluate the patient’s home environment.Assess patients for muscle strength, mobility, gait, ROM potential for rehab.Direct patients and families/caregivers in the use and care of therapeutic appliances.Determine priority needs for physical therapy.Report the patient's reaction to treatment or changes in their condition to their physician.Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.Instruct other personnel and/or family/caregiver the goals of the physical therapy program for the patient by participating in case conferences.May train the patient in the use of prosthetic devices.Identify patient and family/caregiver needs for other home health services and refer as necessary.Prepare and submit clinical and progress summaries based on the attainment of goals.Participate in discharge planning for the patient.Prepare and submit a clinical progress summary based on the attainment of goals as directed by Organization policy.Provide physical therapy consultation to home families/caregivers when indicated.Provide in-service education programs for nursing organization personnel as needed.Participate in the peer consultation process.Supervises PTAs according to organization policy and state regulations.When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two (2) weeks.Collaborate with other team personnel to achieve well-rounded care.Qualifications:Bachelor’s, master's or doctorate in physical therapy from an accredited program.Active license to practice as a physical therapist in the state of Indiana.2 years of previous clinical experience as a physical therapist, preferred.Prior home health or community-based experience is preferred but not required.Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.Why Summit?We offer a well-rounded benefits package to support you both at work and at home.
