Physical Therapist, PT (Healthcare)

Lone Tree Post Acute is a 99-bed skilled nursing facility located in Antioch, California. We are looking for a Physical Therapist to join our team.

Job Responsibilities:

1. Develop effective treatment plans and obtain approval for services from referring physicians.
2. Treat patients according to the physician treatment plan.
3. Assist the nursing department with training of Restorative Aides.
4. Supervise Physical Therapy Assistants in patient care, following state practice act.
5. Communicate with supervisors and health team members regarding patient progress and plans.
6. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
7. Conduct in-service training for staff.
8. Document evaluations, treatment notes, progress reports, and recertifications per PT Board State Practice Act and payer requirements.
9. Record treatment modifications as per policies.
10. Instruct families and nursing staff in maintenance and caregiver training for discharge planning.
11. Participate in discharge planning processes.
12. Recommend and facilitate ordering of durable medical equipment to promote patient independence.
13. Report equipment issues for maintenance.
14. Maintain positive and effective communication with patients, families, staff, and colleagues.
