Job Summary:
The physical therapist assesses, plans, organizes, and participates in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury. Services are provided across the full continuum and are rendered in a cost-conscious, quality-focused, and customer-oriented manner. Compliance with all applicable regulatory standards is also required in this role.
Core Responsibilities and Essential Functions:
- Treatment Planning and Provision of Care
- Implements the physical therapy treatment plan
- Selects appropriate treatment activities to progress patients towards goals
- Revises goals/plan of care with patient/family input
- Provides ongoing patient/family education and training
- Provides discharge instructions, follow-up, and referrals to community resources as appropriate
- Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services
- Patient Assessment
- In collaboration with the physician, responsible for establishing and evaluating treatment programs, plans of care, goals, and discharge plans related to physical therapy needs
- Establishes a plan of care with the patient and/or family
- Develops realistic discharge plans incorporating patient and/or family goals
- Documentation
- Documents evaluation and plan of care according to departmental guidelines
- Documents each treatment session according to protocols
- Completes and submits family education, discharge, and other required documentation within facility and program guidelines

Minimum Education:
Doctorate, Masters, or Bachelors in Physical Therapy
Licenses and Certifications:
- Physical Therapist license
- Basic Life Support (BLS) or BLS Instructor
