Core Responsibilities and Essential Functions

• Treatment Planning and Care: Implements treatment plans, selects appropriate activities, revises goals with patient/family input, provides education and discharge instructions, and acts as a referral source.
• Patient Assessment: Collaborates with physicians to establish and evaluate treatment programs, goals, and discharge plans, involving patients and families.
• Documentation: Records evaluations, treatment sessions, and family education according to departmental protocols, ensuring timely submission.
• General Requirements: Upholds ethical practices, maintains confidentiality, punctuality, dress code compliance, effective communication, and participates in meetings and training.
• Customer Service: Demonstrates AIDET principles, fosters a positive environment, and shows respect to patients and staff.
• Professional Development: Engages in ongoing education, completes required competencies, and participates in orientation and mandatory activities.

Minimum Education

• Doctorate, Masters, or Bachelors in Physical Therapy

Licenses and Certifications

• Physical Therapist license
• Basic Life Support (BLS) or BLS-Instructor certification

Experience and Skills

Effective communication, customer service orientation, teamwork, and basic computer skills are required.
