Job Overview

We have an exciting opportunity for a Per Diem (PRN) Physical Therapist. We are seeking a PT to join our in-house team of dedicated professionals providing person-centered care to our residents, aiming for positive outcomes.

Responsibilities
  1. Consult with patients to assess their physical condition.
  2. Interpret patient evaluations and test results.
  3. Develop and implement treatment plans using various techniques.
  4. Create personalized rehabilitation programs.
  5. Administer prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
  6. Ensure treatments follow the established care plan.
  7. Monitor patient responses and adjust treatments collaboratively with the interdisciplinary team.
  8. Provide education and support to patients, families, and caregivers.
  9. Collaborate with team members to ensure quality care.
  10. Complete thorough and timely documentation complying with regulations.
  11. Attend meetings as required by the Director of Rehab.
Skills and Qualifications
  • Preferred experience in SNF/long-term care.
  • Up-to-date knowledge of treatment practices.
  • Knowledge of Net Health Electronic Documentation system preferred.
  • Ability to manage patients with diverse personalities.
Requirements
  • Active and valid Texas Physical Therapy license.
  • Good standing with regulatory agencies and licensing boards.
