Position Details: PRN Physical Therapist at Francis Marion Healthcare and Rehabilitation in Marion, VA. Managed by Ahava Healthcare since 5/1/2025, this 109-bed skilled nursing facility offers short-term rehab services.

Responsibilities:

1. Review resident information and evaluate its impact on assessment and treatment.
2. Conduct thorough functional assessments using standardized procedures.
3. Develop and implement individualized treatment plans with achievable goals.
4. Maintain timely and accurate documentation of evaluations, treatments, and progress.
5. Supervise and instruct assistants in treatment procedures.
6. Document treatment sessions and revise plans as needed.
7. Provide discharge summaries and orient residents to services and exercises.
8. Collaborate with nursing staff, residents, and families to optimize functional abilities.
9. Refer residents for additional services post-discharge and stay updated on community resources.
10. Participate in team conferences, meetings, and quality improvement activities.
11. Ensure compliance with regulatory standards, facility policies, and reimbursement guidelines.
12. Support the company's mission, demonstrate professionalism, and work independently with appropriate supervision.

Qualifications:

• Bachelor's or Master's degree in Physical Therapy.
• Current state license.
• BLS CPR certification required at hire or during orientation; must remain current.
