Job summary

WeCare Centers at The Green Home Care Center in Wellsboro, PA seeks a Physical Therapist to help residents improve mobility and enable them to perform daily life activities independently. This as-needed position welcomes new graduates and offers competitive compensation plus comprehensive benefits.

The therapist will work within our team to deliver care per an established plan and support residents and families.

Qualifications

• Valid state license as a Physical Therapist.
• Experience in a skilled nursing environment preferred.
• New graduates are welcome to apply.
• Ability to communicate clearly and instruct patients and families.

Responsibilities

• Plan and administer treatment to residents to relieve pain and improve function.
• Ensure therapy follows the established plan of care.
• Provide clinical support and educate patients, families, and caregivers.

• Complete required documentation per policy and regulations.

Key Responsibilities

• Plan and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Assure all therapy is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Monitor patient response to treatment intervention
• Complete required forms and documentation in accordance with company policy and state/federal regulations

Skills, Knowledge and Expertise

• Valid State qualifying license or certification.
• Experience in skilled nursing environment is preferred.
• New graduates are welcome.
• Ability to communicate and instruct effectively.
• Hands on experience with therapeutic exercises, functional training, and physical modalities.
