Job summary

Alvarado Meadows is seeking a Physical Therapist to evaluate, plan, and administer patient-centered rehabilitation in a structured care setting. You will work closely with an interdisciplinary team to implement plans of care, monitor progress, and adjust therapies as needed to improve function and relieve pain.

Qualifications

• Experience in SNF/long-term care settings preferred.
• Keeps up-to-date knowledge of treatment practices.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to manage patients with diverse personalities.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering prescribed physical therapy treatments to relieve pain and restore function.
• Ensuring all treatment aligns with the plan of care.
• Monitor patient response and adjust treatment with IDT collaboration.
• Provide education to patients, family, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Ensure timely documentation meeting regulatory requirements.
• Attend required meetings designated by the Director of Rehab.

Job description

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
