Job summary

Patriot Homecare is seeking a Full Time Homecare Physical Therapist to provide comprehensive PT services across Fairfield, Hamilton, West Chester, and Mt Healthy. You will perform initial and ongoing assessments, develop treatment plans with physician input, and support an interdisciplinary care approach in patients' homes.

The role requires a PT degree, state licensure, and at least one year of home health experience.

Qualifications

• Must be a graduate of a Physical Therapy program
• Must be licensed or registered by the State
• At least one year of institutional/home health experience
• Criminal background check required
• Current CPR (BLS) certification

Responsibilities

• Provide initial and ongoing comprehensive assessments including OASIS
• Establish and revise treatment plans with physician collaboration
• Develop and implement interdisciplinary patient care plan relevant to Physical Therapy
• Assist physician with evaluating patients using diagnostic and prognostic tests
• Educate and supervise family and patient in the PT program
• Write clinical/progress notes on day of visit and file within 72 hours

The responsibilities of our Physical Therapists include:

• Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments
• Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests
• Teach, supervise, and counsel the family and patient in the physical therapy program
• Write clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.

Job Conditions:

• The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
• Must be licensed or registered by the State
• Must have at least one year of institutional/home health experience
• Must have criminal background check
• Must have a current CPR (BLS) certification
