Overview

The PT, PRN provides prescribed treatments of physical therapy that involve a complex and sophisticated level of modalities designed to restore the client to his/her maximum rehabilitation potential.

Responsibilities

• Performs initial and ongoing physical therapy assessments to determine client functional level, including designated OASIS items for care coordination.
• Provides direct therapeutic physical therapy services as planned and certified by the client’s physician.
• Evaluates equipment needs to enhance client functionality.
• Records and reports client progress to physicians and staff; performs interim evaluations for recertification; assesses treatment outcomes.
• Teaches and supervises nurses, aides, caregivers, and clients to ensure continuity of therapy programs.
• Collaborates with staff on goal setting, participates in clinical conferences, and identifies barriers to outcomes.
• Delivers services aligned with accepted treatment standards tailored to client conditions.
• Assists in developing, writing, and revising the plan of care in collaboration with staff and physicians.
• Consults on discharge planning and notifies the agency of imminent discharges for proper documentation.
• Maintains comprehensive clinical records, documenting limitations and progress.
• Develops maintenance therapy programs and instructs staff and caregivers accordingly.
• Responsible for equipment used by clients.
• Evaluates OASIS items upon discharge for care coordination.

Qualifications

• Current Physical Therapy License in Maryland.
• Graduated from an approved physical therapy program.
• Minimum one year of clinical experience.
• Current CPR certification.

• Ability to work independently and manage multiple priorities.
