Job summary

INNOVATE REHAB AND WELLNESS in Sarcoxie, MO is seeking a licensed Physical Therapist for a PRN role in a Skilled Nursing Facility. You will evaluate patients, develop personalized treatment plans, and lead therapy sessions to restore movement, function, and quality of life with an interdisciplinary team.

This position offers flexible, as-needed scheduling, one-on-one patient care, and a supportive, therapist- and veteran-owned culture.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands‑on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
