Job summary

Buena Vida Nursing & Rehabilitation Center San Antonio is seeking a Per Diem Physical Therapist to join our in-house team serving residents with person-centered care. You will assess patients, design, and implement rehabilitation plans to improve function and comfort.

Responsibilities include thorough documentation, collaboration with the interdisciplinary team, and education for patients and families, with flexible scheduling to meet resident needs in a SNF setting.

Qualifications

• Active Texas PT license in good standing.
• Experience in SNF/long-term care preferred.
• Knowledge of PT treatment practices and documentation systems.

Responsibilities

• Consult with patients to assess their physical condition.
• Develop and implement personalized rehabilitation plans.
• Document thoroughly and in compliance with regulations.
• Collaborate with the IDT and educate patients and families.
• Monitor response to treatment and adjust as needed.
• Attend meetings as designated by Rehab Director.

We are looking for aPTto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
