Job summary

Hireology is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents with person-centered care to achieve positive outcomes.

As a Physical Therapist, you will assess patient needs, develop and implement treatment plans, and collaborate with an interdisciplinary team while ensuring documentation complies with regulations.

Qualifications

• Active Texas license in Physical Therapy.
• In-depth knowledge of physical therapy practices.
• Experience in SNF/Long-term care preferred.
• Familiarity with Net Health ED is a plus.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans and implementing them.
• Creating personalized rehabilitation programs for patients.
• administering therapies to relieve pain and improve function.
• Ensure documentation is completed thoroughly and in compliance with plans of care.
• Monitor patient response and adjust treatment as appropriate in collaboration with the IDT.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality patient care.

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
