In this role, you will assess residents’ needs, develop individualized therapy programs, and deliver care that supports rehabilitation, independence, and improved quality of life.

• Provide high-quality physical therapy care within the PT scope of practice, following established standards, departmental procedures, and productivity expectations.

• Assist in evaluations and develop individualized treatment plans, including target lists, short-term goals, and long-term goals.

• Collaborate with interdisciplinary teams to support each resident’s rehabilitation and overall well-being.

Qualifications

• Licensed Physical Therapist in the state of Pennsylvania

• BLS (Basic Life Support) certification
