Job description

Summary

We are hiring a PRN PT for Home Visits in Ponca City!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps develop a treatment plan, performs interventions to improve patient well-being, and evaluates patient progress.

Responsibilities
  1. Evaluate the patient's functional status, body systems as required for CMS documentation, and physical therapy needs. Consult with the physician to develop the therapy plan of care.
  2. Provide services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  3. Observe and report the patient's response to treatment and any condition changes to the nurse supervisor and/or physician.
  4. Treat patients to relieve pain, restore or maintain function, using accepted standards and evidence-based techniques.
  5. Instruct and inform healthcare team members, the patient, and family/caregivers about the plan of care, limitations, and progress.
  6. Visit patients within the service area and nearby agencies as needed.
  7. Complete a progress note for each visit and submit it per policy.
  8. Complete patient evaluations and develop the PT plan of care within state guidelines. Report outcomes, goals, and projected care frequency within 24 hours.
  9. Participate in staff conferences and committees as requested.
  10. Supervise home health aides per policy and state regulation if permitted.
  11. Supervise the PTA as per policy and regulation.
  12. Maintain schedule flexibility to provide care as needed, including participation in weekend rotations and holiday availability.
  13. Prepare discharge summaries or condition reports at least every 60 days.
  14. Provide in-service education as requested.
  15. Adhere to LHC Therapy Standards of Practice.
  16. Maintain and enhance clinical knowledge and skills, and uphold licensure requirements.
  17. Perform other duties as assigned.
Education and Experience

Licensure Requirements

  • Current Physical Therapy license in Oklahoma
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
