Job summary

The Outlook at Windhaven in Plano, TX is seeking a licensed Physical Therapist to join our dedicated therapy team. You will evaluate, develop, and implement individualized treatment plans to help residents improve mobility and regain independence, collaborating with residents, families, and an interdisciplinary care team.

Qualifications

• Licensed Physical Therapist in the current state.
• Knowledge of therapy industry and relevant regulations.
• Ability to organize, prioritize and complete multiple tasks in a dynamic environment.
• Experience in IL/AL/OP setting or mature population rehabilitation and wellness needs.
• Basic computer skills: Outlook, Word, Excel.

Responsibilities

• Model a service attitude towards others and take initiative to improve care.
• Ensure safety for residents, patients, visitors, and staff.
• Document assessments, reassessments, progress notes, and discharge summaries per policy.
• Educate residents, families and staff on Physical Therapy modalities and care.
• Demonstrate clinical judgment in resident treatments.
• Orients and directs PTAs, Rehab Techs and students.
• Review documentation by PTAs.
• Screen residents to determine benefit from PT interventions.
• Develop and participate in PT programming for residents.
• Attend in-services and meetings as required.
• Maintain continuing education per licensure requirements.
• Other duties as assigned by the Director of Rehab.

ESSENTIAL DUTIES:

• Model a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients, visitors, and co-workers
• Ensure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
• Document all assessments, reassessments, progress notes, discharge summaries and other clinical work in the medical record per departmental policy and procedure
• Provide resident, family and staff education pertaining to Physical Therapy modalities to ensure understanding and proficiency of care
• Demonstrates good clinical judgment in all aspects of resident treatments in which trained to provide
• Orients, instructs and directs work activities of Licensed Physical Therapy Assistants, Rehab Techs and Physical Therapy students
• Reviews documentation written by the Licensed Physical Therapy Assistants
• Screening residents to determine their potential benefit from physical therapy intervention
• Develops, directs and/or participates in physical therapy programming for the residents
• Attends in-services and other meetings as required on an ongoing basis
• Maintains continuing education per state licensure requirements
• Other duties as assigned by the Director of Rehab

MINIMUM QUALIFICATIONS:

• Licensed Physical Therapist in the current state
• Knowledge of therapy industry and federal, state and local government regulations
• Demonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment
• Demonstrated efficiency strategies for fast paced environment
• Experience in IL/AL/OP setting or mature population rehabilitation and wellness needs
• Basic computer skills, Outlook, Word, Excel
• Genuine caring for and interest in the care of mature adults, handicapped people and hospice Patients
• Ability to perform Essential Duties
• Ability to perform work tasks within the Physical Demand Requirements
