Job summary

Northshore Home Health Care is seeking a Physical Therapist for home health, PRN. You will evaluate patients, develop and implement treatment plans, and administer interventions to improve function in patients' homes.

In collaboration with physicians and the healthcare team, you will monitor progress, document outcomes, and educate patients and families about care plans. The role emphasizes adaptability to varied home settings and strong communication skills.

Qualifications

• Current Physical Therapy license in the state of practice.
• Valid driver's license, vehicle, or reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days.
• Strong assessment, communication, and patient care skills.
