Job summary

DayToDay Health seeks a licensed Physical Therapist to provide home health services, assessing patients’ functional status, mobility, endurance, and pain, and implementing tailored treatment plans.

You will work with physicians and the care team, document progress, educate patients and families, and ensure adherence to state guidelines while maintaining clinical excellence and ongoing professional development.

Qualifications

• Licensed in Physical Therapy in the relevant state.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Evaluate patients' physical and systemic health, collaborating with physicians on care plans.
• Provide therapy within legal and professional scope, coordinating with healthcare team members.
• Observe and report patient responses and condition changes.
• Administer treatments to relieve pain, restore, or maintain function using evidence-based methods.
• Educate patients, families, and healthcare team members about care plans, limitations, and progress.
• Conduct patient visits across service areas, documenting progress and outcomes.
• Complete evaluations and develop individualized plans of care per state guidelines.
• Participate in staff meetings and committees as needed.
• Maintain clinical knowledge and licensure through ongoing education.
