Job Summary:

We are committed to providing exceptional care in a skilled nursing environment. We seek a skilled and motivated Physical Therapist to assess, plan, and implement individualized therapy programs, working closely with a multidisciplinary team to promote functional independence and well-being.

Work Environment: Clinical healthcare setting.

Responsibilities:

1. Evaluate patients’ physical conditions, mobility, and abilities to develop treatment plans.
2. Create and implement personalized therapy programs.
3. Administer therapeutic exercises, manual therapy, and modalities.
4. Monitor and document patient progress, adjusting treatments as needed.
5. Educate patients and families on exercises, safety, and home care.
6. Collaborate with healthcare professionals to ensure coordinated care.
7. Maintain accurate documentation in compliance with policies and regulations.
8. Follow safety and infection control protocols.

Qualifications:

• Doctoral or Master’s degree in Physical Therapy from an accredited institution.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing or long-term care setting preferred, but not required.
