Job summary

Rockwood Manor is seeking a dedicated Physical Therapist for a per diem/PRN role in Texas. You will assess patients, design and implement rehab plans, and work with an interdisciplinary team to achieve positive outcomes.

The position emphasizes patient education, documentation, and collaboration with clinicians to ensure quality care. A Texas PT license and experience in long-term care are preferred.

Qualifications

• Active/Valid Texas license in Physical Therapy.
• In good standing with regulatory agencies.
• Experience in SNF/long-term care is preferred.

Responsibilities

• Consult with patients to assess their physical condition.
• Interpret patient evaluations and test results.
• Develop and modify treatment plans using various techniques.
• Create personalized rehabilitation programs for patients.
• Administer PT treatments to relieve pain and restore function.
• Ensure all treatments align with the plan of care.
• Monitor patient progress and adjust treatment with IDT input.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete documentation per company and regulatory requirements.
• Attend meetings designated by the Director of Rehab.

We are looking for aPTto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
