Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents with person-centered care to achieve positive outcomes.

This PT role requires an active Texas license and prefers SNF/long-term care experience, with familiarity in Net Health EHR. Flexible scheduling and collaborative work with an interdisciplinary team are offered.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to manage patients with diverse personalities.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering prescribed physical therapy treatments to relieve pain and improve function.
• Ensuring all treatment aligns with the plan of care.
• Monitoring patient responses and adjusting treatments with IDT collaboration.
• Providing education to patients, families, and caregivers.
• Collaborating with the interdisciplinary team to ensure quality patient care.
• Documenting thoroughly to meet regulatory requirements.
• Attending required meetings designated by the Director of Rehab.

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
