Job summary

LiveWell Homecare Agency, LLC is seeking a PRN Physical Therapist to provide professional PT services to patients in their homes across Texas. You will evaluate patients, design and implement individualized treatment plans, and participate in re-evaluations and discharge planning.

The ideal candidate holds a Texas PT license (or temporary license), BLS, and at least 1 year of licensed PT experience. Strong communication and teamwork are essential for quality patient outcomes.

Qualifications

• Graduate of an accredited baccalaureate or post-baccalaureate Physical Therapy program.
• Current Basic Life Support Certification.
• Valid Texas Physical Therapy license or temporary license.
• At least 1 year of licensed Physical Therapy experience.

Responsibilities
