Job summary

Choice Health at Home is seeking a Physical Therapist PRN to support Durant, OK home health patients. The role emphasizes comprehensive rehab plans, accurate initial exams, and ongoing program modification to maximize mobility and reduce pain.

You will ensure safety, documentation, and collaborative communication with families and care teams. The position requires an accredited PT program completion, state licensure, CPR, and a valid driver’s license.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Must pass state licensure exam.
• 1 year home health experience preferred.
• CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs to improve mobility, relieve pain, increase strength, and correct disabling conditions.
• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan and carry out individually designed programs to maintain, improve, or restore physical function.
• Evaluate effects of treatment and adjust therapies to maximize benefit.
• Maintain infection control/universal precautions in patient care and ensure continuity of quality care.
• Provide effective communication to patient, family, team members, and other health care professionals.
• Promote agency philosophy and administrative policies and complete all required documentation.

Position Summary

Assess , plan, organize , and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury .

As a Physical Therapist , you will:

Perform and document an initial exam , evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control/universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient/family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license

1 year of home health experience preferred

CPR certification

Valid Driver’s license

Valid Auto Insurance

HomeCare HomeBase experience preferred

Physical Demands
