Job summary

Choice Health at Home in Norman, OK is seeking a Physical Therapist (PT) PRN to join our Home Health team. You will evaluate patients, develop personalized treatment plans, and help patients regain independence in the comfort of their homes.

This role offers flexible scheduling, autonomy, and opportunities to make a lasting impact on patients' lives. Requirements include graduation from an accredited PT program, active PT license, CPR certification, and a valid driver's license.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license.
• CPR certification.
• Valid driver's license and auto insurance.
• Home Health experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Evaluate patients and develop personalized treatment plans.
• Improve mobility, strength, balance, and overall function.
• Educate patients and caregivers on exercises, safety, and disease management.
• Monitor progress and adjust treatment plans to maximize outcomes.
• Collaborate with physicians, nurses, and interdisciplinary team members.
• Complete timely and accurate clinical documentation.
