Job summary

Tennessee Quality Care is seeking a PRN Physical Therapist to join our Home Health team in Camden, TN and surrounding counties. You will provide one-on-one skilled PT services in patients’ homes, helping them regain mobility and independence.

Responsibilities include evaluating patients, creating individualized plans, educating patients and families, and coordinating with the care team. CPR-certified, APCP-licensed, and valid driver’s license required; Home Health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the APTA.
• Current Physical Therapist license in the state of practice.
• Home Health experience preferred, but not required.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and current automobile insurance.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in the patient’s home.
• Help patients improve mobility, strength, balance, and overall functional independence.
• Educate patients, families, and caregivers on treatment plans, home exercises, and safety strategies.
• Recommend adaptive equipment and home modifications to promote safety and independence.
• Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
• Accurately document evaluations, progress notes, and patient outcomes in a timely manner.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.
• Encourage and support patients throughout their rehabilitation journey.
