Responsibilities

1. Perform physical therapy evaluations and treatments based on physician orders, including recommendations and procurement of adaptive equipment.
2. Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and/or function.
3. Maintain communication among the treatment team, physician, patient, and family members as applicable.
4. Maintain thorough and timely documentation of services provided for proper billing.
5. Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities, in accordance with state practice acts.

Qualifications

1. Minimum Bachelor’s Degree in Physical Therapy from an accredited program.
2. Current, unrestricted Physical Therapist license in the state of operation.
3. Current BLS or CPR certification in accordance with state regulations.
4. Minimum one (1) year of PT experience preferred.
