Job summary

Palm Garden Rehab in Clearwater is seeking a dedicated Physical Therapist (PT) to assess residents, develop and update care plans, and guide exercises to restore function after illness or surgery.

You will collaborate with the clinical team, document progress in the medical record, and help residents achieve safety and mobility goals. Florida PT license required; skilled nursing or rehab experience is a plus but not required.

• Active Florida PT license.
• Caring attitude and strong teamwork.
• Experience in skilled nursing or rehab is a plus but not required.

• Assess new residents and develop care plan. Update therapy plans based on their recovery, safety needs and goals.
• Guide residents in exercises, mobility training, strengthening, balance work and fine motor activities to help residents regain function after illness, injury or surgery
• Document progress, challenges, and improvements in the medical record and communicate updates to the clinical team.

What We Are Looking For

• Active and Clear PT license in the State of Florida
• A caring attitude and strong teamwork skills
• Skilled nursing or rehab experience is a plus but not required
