Job summary

Palm Garden is seeking a dedicated Physical Therapist (PT) to provide personalized therapy services in a home health setting. You will work with residents to improve independence and quality of life, documenting care and communicating changes using WellSky. This role emphasizes compassionate teamwork and positive resident experiences.

Active PT licensure is required; Home Health experience is a plus. Palm Garden offers a supportive environment focused on resident-centered care and teamwork.
