Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluates the patient's functional status and therapy needs, assisting in developing the care plan with the physician.
• Observes and reports patient responses and condition changes to the supervising nurse and/or physician.
• Treats patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
• Instructs and informs healthcare team members, the patient, and family/caregivers about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
