Job summary

Matagordanr is seeking a Per Diem Physical Therapist to join our in-house rehab team serving residents with person-centered care. You will assess patients, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to ensure quality outcomes.

Active Texas PT license and experience in SNF/long-term care are preferred. In-house rehab options, flexible schedules, and competitive compensation are offered to support optimal patient care.

Qualifications

• Experience in SNF/long-term care settings is preferred.
• Strong knowledge of current physical therapy treatment practices.
• Familiarity with Net Health Electronic Documentation System preferred.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of techniques.
• Creating personalized rehabilitation programs for patients.
• Administering physical therapy treatments to relieve pain and restore function.
• Ensure treatment aligns with the plan of care and regulatory requirements.
• Monitor patient progress and adjust plans as needed in collaboration with the IDT.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete documentation thoroughly and timely per company and state/federal rules.
• Attend meetings as designated by the Director of Rehab.

We are looking for aPT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
