Job summary

Amistad Nursing and Rehabilitation Center, LLC is seeking a Per Diem Physical Therapist to join our in-house rehab team, delivering person-centered care to help residents achieve positive outcomes.

You will assess patients, develop and implement treatment plans, administer PT therapies, document thoroughly, and collaborate with the interdisciplinary team to ensure quality care. New grads are welcome, with flexible schedules. Active Texas PT license required; knowledge of Net Health EHR is a plus.

Qualifications

• Active Texas license in Physical Therapy.
• Knowledge of current treatment practices and rehab methods.
• Experience in SNF/long-term care settings preferred.
• Familiarity with Net Health EHR is a plus.

Responsibilities

• Consult with patients to assess their physical condition.
• Develop and implement individualized rehabilitation plans.
• Administer PT treatments to relieve pain and improve function.
• Document care thoroughly and in compliance with regulations.
• Collaborate with the rehab team to ensure quality care.
• Attend required meetings with the Director of Rehab.

Wehave an amazing opportunity for aPer Diem (PRN) Physical Therapist!

We are looking for aPT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
