Job summary

St. Rose Hospital in Hayward, CA is seeking a Per-Diem Physical Therapist for the AM shift to provide therapy services, evaluate patients, plan care, and deliver home programs under supervision.

Qualifications

  • License/Certification: California Physical Therapy license or eligible.
  • Current BCLS from American Heart Association required.
  • BA or MA in Physical Therapy required.
  • Proof of all required licenses/certifications due at hire.
  • Experience: 6 months to 2 years preferred; new grads may be acceptable.

Responsibilities

  • Provide therapy services, including evaluations and planning patient care programs.
  • Design and revise plans of care and goals for each patient.
  • Document, report to physicians, and ensure timely charting.
  • Submit accurate charge sheets daily and obtain physician orders.
  • Supervise aides, PT assistants, students, and volunteers as assigned.
  • participate in education programs and quality improvement activities.

Job description/Responsibilities:
  • Per-Diem AM Shift APPROXIMATE PAY RANGE: $65.05 - $80.00 Under general supervision is responsible for providing therapy services to patients; including evaluations, planning individual patient care programs, treatments, and home programs.
  • Performs patient evaluation and administers treatment
  • Designs and revises plans of care and goals
  • Provides written and/or verbal reports to physicians
  • Documentation is complete and timely and meets unit standards and regulatory requirements
  • Completes accurate charge sheets for patients treated and submits them daily
  • Obtains physician orders for evaluation and treatment.
  • Ensures proper authorization is obtained, when required prior to treatment
  • Participates in in-service education programs
  • Demonstrates flexibility in meeting hospital and unit needs
  • Participates in clinical instruction of affiliating students
  • Provides instruction, and delegates to aides and physical therapy assistants, new employees, PTLA's students, and volunteers
  • Keeps abreast of current trends and developments in Physical Therapy and Rehabilitation
  • Participates in related activities such as patient care committees, case conferences, personnel orientations, community education projects, and in-service education
  • Demonstrates interest in personal and professional growth
  • Participates in development and evaluation of clinic standards, budget, and quality improvement indicator
  • Performs pre-employment fitness testing
  • Performs worksite evaluations and worksite training
  • Performs wound care
  • Participates in performance improvement activities
  • Performs other job-related duties as assigned by the Director, Rehabilitation/Occupational Services
Required Qualification:
  • License/Certification Current California Physical Therapy License or eligible
  • Current BCLS from the American Heart Association required

  • Education BA or MA in Physical Therapy required.
  • Proof of all required educational levels due at time of hire
  • Experience Six months to two years work experience preferred. New graduates may be acceptable
  • Skills - Knowledgeable of the principles and practices of Physical Therapy and other disciplines
  • Ability to evaluate, plan and carry out a Physical Therapy and Rehabilitation program for individual patients
  • Ability to explain, demonstrate and teach physical and rehabilitative measures
  • Ability to supervise support personnel
  • Ability to work effectively with others in an inter-disciplinary team relationship
  • Ability to be self-directing and to seek direction and consultation as needed
