Job Description

Overall Responsibility: Administers therapy services to patients requiring intermittent physical therapy service and teaches and/or supervises the family and other members of the home health team. These services are performed in accordance with the physician’s orders.

  • Supervise/training personnel such as PTAs or HHAs
  • Evaluate patient within 72 hours of written referral
  • Provide therapy services as outlined in the Plan of Care
  • Plan, administer, and supervise PTA and HHA in accordance with treatment plan
  • Communicate with referring physician and rehabilitation team regarding the patient’s total treatment program
  • Maintain accurate daily records of treatments given to patients
  • Provide in-service education to facilities, support staff, students, and community
  • Follow established policies and procedures
  • Chart services rendered, note changes in the patient’s condition and/or family and home situation, make revisions in the care plan as needed, record supervisory visits, evaluate patient care and progress, and close charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders
  • Maintain confidentiality of client’s situation, records, and services provided according to HIPAA regulations
  • Evaluate effectiveness of therapy service to the individual and family
  • Consult with attending physician concerning alterations of the plan of treatment
  • Submit all patient notes and/or visit records on a timely basis
  • Discuss with the Director of Clinical Services problems concerning patients and how these may best be resolved
  • Discuss changes in patient needs
  • Discuss involvement of other members of health team
  • Obtain orders for paramedical services and submit referrals for nursing, medical social services, etc.
  • Coordinate total patient care
  • Cooperate with other agencies providing nursing or related services to provide continuation of care and implement a comprehensive care plan
  • Continuously strive to improve therapy care by broadening and deepening knowledge through formal education, workshops, conferences, active participation in professional organizations and reading
  • Participate in interdisciplinary team conferences
  • Attend on-the-job and in-service training
Qualifications
  • Licensed Physical Therapist in the state of California
  • Preferred: Physical therapy experience
  • Physical activity requirement within 6 months prior to first patient contact
  • Current TB results (within last 12 months)
  • Current CPR Certification
  • Valid driver’s license
